Haryana: The Director of Medical Education (DME Haryana) has begun the physical document verification of MBBS/BDS courses for round 1.
The detailed schedule –
10th November 2022 –
1. Open Category – 9:00 am
2. Minority Category – 2:00 pm
12th November 2022 –
Management Category – 9:00 am
Venue - PWD Rest House, Near Forest Office, Gurugram-122001
The candidates are requested to read the public notices UHSR/Acad./2022/A-11(/8669 dated 18.10.2022 & UHSR/Acad./2022/A-III/SPL dated 20.10.2022 regarding the list of documents to be required at the time of physical documents verification (Annexure I & II). And check the admission web portal (www.uhsrugcounselling.com) and the website of Pt.B.D. Sharma UHS Rohtak (www.uhsr.ac.in), SMS and emails regularly for any latest updates.
Documents to be brought for verification –
1. Matriculation Certificate/High School/ICSE/Class X/Class Xli or its equivalent certificate as proof of date of birth
2. Three copies of the recent photographs (passport size) self-attested used for the NEET form.
3. Character certificate of the Head of Institution last attended/First Class Executive Magistrate.
4. Resident certificate issued by the competent authority of the State Government of Haryana (if applicable'bonafide resident certificate issued by the competent authority of the State Government ( if applicable) as per Govt. of Haryana instruction dated 12.10.2022.
5. Caste Certificate issued by the competent authority of the Haryana State Government (if applicable) as per Haryana Govt. notification dated 12.10.2022.
6. Certificate of Dependent of Freedom Fighter Certificate issued by the competent authority of the Haryana State Government (if applicable) as per Haryana Govt. notification dated 12.10.2022.
7. Certificate of ESM priority wise issued by the competent authority of the Haryana State Government (if applicable) as per Haryana Govt. notification dated 12.10.2022.
8. Creamy / Non-Creamy certificate for BC-A / BC-B category candidates as per Govt. of Haryana notification dated 17.11.2021 certificate for the year 2022-2023 should be brought at the time of Ph} social document verification (as per Haryana Govt. notification dated 12.10.2022).
9. Income and Asset certificate for EWS category candidates issued by competent authorit} as per Haryana Govt._ Notification No. 16/6/2022-6HB dated 12.10.2022 certificate issued as per notification Govt. of Haryana Notification No. 16/6/2-22-6HB dated 12.10.2022 for the year 2022-23 (as per Haryana Govt. notification dated 12.10.2022).
10. Certificate of Disability issued by notified centres by NTA or Medical Board PGIMS, Rohtak (if applicable as per Haryana Govt. notification dated 12.10.2022.
11. Result card of NEET —UG 2022.
12. As proof of identification Aadharr card/PAN card/Driving license/Passport/Voter ID/Ration CardiParivar Pehcahan Patra.
13. Printout of the Application form.
